Please note that we cannot serve your client or add them to our waitlist until all three steps of the referral are complete:

STEP ONE: COMPLETE EAHR CONSENT PAPERWORK

PLEASE CAREFULLY REVIEW THE PACKET OF CONSENT FORMS LINKED BELOW WITH YOUR CLIENT AND OBTAIN THEIR SIGNATURE.

Our program's required consent paperwork is available in English, French and Spanish. (Dari and Pashto coming soon!) Even if your client is fully literate in the language of the consent paperwork and comfortable filling it out themselves, please ensure you have a direct conversation with your client about the role of the evaluation in their immigration case and confirm their informed consent to participate in the EAHR program before submitting a referral. STEP TWO: SHARE CASE DOCUMENTS WITH EAHR TEAM

REFERRAL DOCUMENTS TO UPLOAD

REQUIRED: We cannot add your client to our services waitlist without these documents.

  1. Signed consent forms

  2. Complete draft of your client's declaration

OPTIONAL: Please additionally upload any other available documents that are highly relevant to the requested evaluation(s).

  • Medical or mental health records

  • Police reports

  • RFEs or NOIDs

STEP THREE: SUBMIT REFERRAL FORM

Your responses on this referral form guide the process of arranging a volunteer evaluator match and preparing for the evaluation interview. We do not coordinate direct phone calls between evaluators and legal representatives, so the information you provide here about the immigration case is especially important.

AFTER SUBMISSION

After you complete the three steps outlined above, the EAHR Support Team will reach out to confirm receipt of the referral and next steps for your client's case.
